In House Dental Plan

At Bayport Dental Care, we want to bring affordable and quality dental care to our community. Our uninsured patients have the option of signing up for our in-house dental plan bringing an affordable option for all of their dental needs.

For our patients without insurance, coverage includes:

• No Deductibles
• No Yearly Maximum
• No Pre-Authorizations
• No Wondering what procedures insurance will cover
• No Waiting Periods
• Cosmetic Dentistry Included

(Please note all family members must live in the same household)

All of your membership information will be kept in your electronic chart. Your plan will be effective the day you sign up and your renewal date will be on the same date the following year. This plan covers dental services ONLY. Additional products will be at a discounted cost.

TERMS & LIMITATIONS

• This is a Dental Discount Plan and is NOT Dental Insurance. It cannot be combined with any other Dental Insurance.
• This plan is good only at Bayport Dental Care. If you are referred to a specialist, they will NOT honor this plan.
• Should there be dental treatment needed following any type of injury requiring you to use disability or workman’s compensation type insurances, this discount plan cannot be used.
• This plan is non-transferrable: Family members cannot be substituted in for another family member.
• This plan is non-refundable: NO refunds will be given if patient chooses not to use their dental plan.
• Rates are subject to change annually.
• Payments for discounted dental services are due at the time services are rendered.
• This offer cannot be combined with any other offers.
• Patients MUST stay on this discount plan for the duration of their full Clear Aligners Treatment.  

PROGRAM DISCLAIMER

The fees outlined in the Dental Savings Plan option or other established fees by Bayport Dental Care are non-refundable. All plan fees are due at the time of Enrollment. An additional fee may be charged for any missed, cancelled or broken appointment without 48 hours prior notice. This plan is non-transferable by the patient. This plan cannot be combined with any other insurance, dental plan, coupon or discount. Failure to comply with the terms of the plan may result in termination of the plan and forfeiture of any fee paid by the patient or other third-party. Bayport Dental Care reserves the right to refuse treatment and/or terminate the patient’s participation in this plan with 30 days written notice, if the patient’s account becomes delinquent or patient is non-compliant. This plan may be modified, amended or cancelled at any time with 30 days prior written notice and may be subject to other terms and conditions. Plan participants are responsible for scheduling their periodic treatments and services. Services not utilized at the end of each year’s membership period are not carried over to the following year. Discounts apply only to services rendered by Bayport Dental Care and does not include treatment for services provided elsewhere even if the patient is referred to a specialist by Bayport Dental Care.
